to get you back home and enjoying life before you know it.

Focus on the Fracture
Orthopedic recovery doesn’t end until well after the discharge from the hospital. Richland Bean-Blossom Healthcare Center, as a team, focuses on the needs of the traumatic hip patient healing. Our team is prepared to implement our recovery PATH for patients that have had a surgical intervention to repair a traumatic hip fracture. Our goal is to have the patient prepared to return to their home or assisted living center within 14 days of arriving at Richland-Bean Blossom.
About Post Acute Transition to Home (PATH) Program
The Post Acute Transition to Home Unit (PATH) assists patients as they transition from a stay in the hospital to our center and then back to home. The PATH is a short-term, post-acute care stay at Richland Bean-Blossom Healthcare Center. Upon arrival, our care team assesses every individual and sets goals for discharge.
The goal of the PATH is to optimize an individual's quality of life and to help the patient transition out of the hospital. While at Richland Bean-Blossom, you'll take part in rehab therapy in our onsite gym with equipment that will help you recover from a hip fracture.
